Completed Cornerstone Adult Medical History Form (link to a pdf) or Cornerstone Pediatric Medical History Form (pdf) and New Patient Forms (pdf).

A prescription for physical therapy if you have been given one by your doctor (please note that a prescription is not necessary prior to your first physical therapy visit, but if you have one, please bring it along with you)

A: You should wear appropriate exercise attire including loose-fitting, comfortable clothes and sneakers.

From the South:
Follow 270 North to exit 31 B (Rt 85 South toward Buckeystown). Bear Right off the exit onto Rt 85 South. Go 0.5 miles and turn Right onto Crestwood Boulevard. Go 0.1 miles and turn Right at the traffic light onto Westview Drive. Go 0.4 miles and turn Left into the 5300 Westview Drive parking lot. Upon entering turn right and follow around to the back of the parking lot. There is a blue Cornerstone sign between buildings 400 and 100 pointing up the path to Suite 108. Park near the sign. We will be on the left as you walk up the path.
From the North:
Follow 270 South to exit 31 B (Rt 85 South toward Buckeystown). Bear Right off the exit onto Rt 85 South. Go 0.4 miles and turn Right onto Crestwood Boulevard. Go 0.1 miles and turn Right at the traffic light onto Westview Drive. Go 0.4 miles and turn Left into the 5300 Westview Drive parking lot. Upon entering turn right and follow around to the back of the parking lot. There is a blue Cornerstone sign between buildings 400 and 100 pointing up the path to Suite 108. Park near the sign. We will be on the left as you walk up the path.

Follow 70 West to exit 53 A (270 South toward Washington). Merge onto 270 South. Go 1.7 miles and take exit 31 B (Rt 85 South toward Buckeystown). Bear Right off the exit onto Rt 85 South. Go 0.5 miles and turn Right onto Crestwood Boulevard. Go 0.1 miles and turn Right at the traffic light onto Westview Drive. Go 0.4 miles and turn Left into the 5300 Westview Drive parking lot. Upon entering turn right and follow around to the back of the parking lot. There is a blue Cornerstone sign between buildings 400 and 100 pointing up the path to Suite 108. Park near the sign. From the West:
